World 4-1

There's not much new here. At least with this stage, there's only one way to go, and that's straight to the right. Of course, you'll be going up and down and all around in order to do it.

World 4-2

This time around, you're going through an underwater maze. Once again, you're going up and down and all around, but you're not just relegated to going all the way to the right like in a Sonic The Hedgehog level.

World 4-3

The boss here is some guy in a clam-like object that floats back and forth across the screen. Attack him as soon as he opens the other half, attack his head and take out the enemies that he fire out of his noggin. After enough damage, he'll change color, indicating that he is near death.

The secret item is in the main underwater section. When you start seeing the breakable blocks to your right, go left instead. You'll be able to find it easily by following the path to the end.
