For the good ending, pick B ("Truthfully, I don't like you.") followed by the normal emotion. Miho starts fearing the worst. Pick A ("I'm sorry...I mean to say I love you!") followed by the sad emotion. Pick B ("We should go out separate ways"), and this time around, you'll get a new option. Pick it, and you and Miho will finally kiss. Press A twice, and you'll be back at the loading screen. You'll meet everyone in the school just like in the blue ribbon ending, but it gets a little different later on.
This time around, you'll get a different phone number from the one you got in the blue ribbon ending. However, instead of ending the game and going back to the main menu, you'll be at the rooftop of the school. Don't select any options; instead, watch the sun set. Miho will show up disguised as Mizuho again. You'll have one more conversation with Miho, but before the two of you part ways, a horde of media photographers and fans show up, having seen through her disguise. They're now coming for both Miho and you, and you both run. As you go through the dialogue, the media and the fans start catching up and Miho trips and falls. A man on a motorcycle shows up, stops and removes his helmet. It's Masaomi! Masaomi tells you to get on so he can help you get away. The credits roll.
Here's the thing, though. If you waited for Miho after talking to Erica and the Typhoon Trio, you get Masaomi in the ending. If you picked the first option to go back to the classroom, picked the fourth option and used the photo of Miho on Sadakachi twice before talking to him, he'll ask if you want to help him clean his bike after school. Pick the first option to say yes. You'll leave the classroom and run into Miho, realizing you were to wait for her. If you make it to the good ending from here, Sadakichi will be behind the helmet instead of Masaomi. The credits roll.
